Preventive Orthodontic Treatment

Preventive orthodontic treatment is a method used to detect and prevent potential jaw and dental development issues at an early age. It is typically applied between the ages of 6 and 12 and aims to prevent more serious orthodontic problems in the future.

Goals of Preventive Orthodontic Treatment

✅ Ensuring proper alignment of teeth
✅ Guiding jaw development
✅ Reducing the need for tooth extraction or jaw surgery
✅ Eliminating bad habits such as thumb sucking and tongue thrusting
✅ Creating a shorter and more effective orthodontic treatment process

Temporary Anchorage Devices (TADs)

Temporary Anchorage Devices (TADs) are small titanium screws used in orthodontic treatment to control tooth movement and prevent unwanted shifts. TADs are placed directly into the gum or jawbone and are used in combination with orthodontic appliances or braces.

Purposes of TADs in Orthodontic Treatment

✅ Facilitating the movement of teeth in the desired direction
✅ Reducing the duration of orthodontic treatment
✅ Enabling more complex jaw and tooth movements
✅ Increasing non-extraction treatment options
✅ Minimizing the need for jaw surgery

Class 1, 2, and 3 Malocclusions

Malocclusion is an orthodontic problem caused by the misalignment of teeth or an improper bite relationship between the upper and lower jaws. According to Angle’s classification, malocclusions are categorized into three main types:

1️⃣ Class I Malocclusion (Normal Bite + Crowding)

✅ The upper and lower jaws are in a normal position, but teeth may be crowded, rotated, or misaligned.
✅ The jaw relationship is normal, but the alignment of teeth is irregular.
✅ It can be easily corrected with orthodontic treatment.

💡 Treatment:

  • Corrected using braces or clear aligners.

  • Tooth extraction may be necessary depending on the severity of crowding.

2️⃣ Class II Malocclusion (Overjet – Upper Jaw Forward)

✅ The upper jaw (maxilla) is positioned forward, while the lower jaw (mandible) is set back.
✅ Creates a “rabbit teeth” appearance, where the upper front teeth protrude.
✅ The lower lip may appear recessed, leading to an imbalanced facial profile.

🔹 Class II, Division 1: Upper front teeth are tilted forward (increased overjet).
🔹 Class II, Division 2: Upper front teeth are tilted inward.

💡 Treatment:

  • In children, functional appliances can guide jaw development.

  • In adults, braces or surgical intervention may be necessary.

3️⃣ Class III Malocclusion (Underbite – Lower Jaw Forward)

✅ The lower jaw (mandible) is positioned ahead of the upper jaw.
✅ Also known as a “reverse bite,” where the lower teeth overlap the upper teeth.
✅ The chin may appear prominent, causing facial asymmetry.
✅ Genetic predisposition plays a significant role.

💡 Treatment:

  • Early intervention with orthodontic appliances can help modify jaw growth.

  • Severe cases may require orthognathic surgery in adulthood.

Clear Braces (Aesthetic Brackets)

Clear braces are orthodontic brackets made from tooth-colored or transparent materials, offering an aesthetic alternative to traditional metal braces. Made from ceramic, sapphire, or composite materials, they are less noticeable while providing the same effectiveness as metal braces.

🦷 Advantages of Clear Braces

✅ Aesthetic Appearance – Much less noticeable compared to metal braces.
✅ Effective Orthodontic Treatment – As strong as metal brackets and equally effective in aligning teeth.
✅ No Discoloration – Sapphire and ceramic brackets do not stain or yellow over time.
✅ Suitable for All Ages – An ideal choice for both adults and teenagers.

What is the Damon System and Self-Ligating Brackets?

The Damon System is a modern orthodontic treatment that uses self-ligating brackets instead of traditional braces with elastic or metal ties. These special brackets have a built-in sliding mechanism that holds the wire in place, reducing friction and making the treatment more comfortable.

🦷 Advantages of Damon Brackets

✅ Faster Treatment Time – More controlled and efficient tooth movement compared to traditional braces.
✅ Less Friction, Less Pain – The sliding mechanism allows natural tooth movement with minimal discomfort.
✅ Fewer Adjustment Appointments – Since no elastic bands are needed, patients may require fewer visits.
✅ Improved Oral Hygiene – No elastic ties mean less plaque buildup around brackets.
✅ Reduced Need for Tooth Extraction – The Damon System expands the dental arch naturally, minimizing the need for extractions in some cases.

Invisalign and Clear Aligner Treatment

Invisalign and other clear aligner treatments are removable, transparent appliances that serve as an alternative to traditional braces. These aligners gradually guide teeth into their ideal positions, offering an aesthetic and comfortable orthodontic solution.

🦷 Advantages of Clear Aligners

✅ Nearly invisible – Ideal for those concerned about aesthetics.
✅ Removable – Easy to take out while eating or brushing.
✅ Comfortable – No metal brackets or wires to cause irritation.
✅ Predictable treatment process – Digitally planned with 3D scans.
✅ Faster results – Effective for mild to moderate crowding in a shorter time.

📌 How Does Invisalign / Clear Aligner Treatment Work?

1️⃣ An orthodontist takes a 3D digital scan of the teeth.
2️⃣ Custom-designed clear aligners are produced.
3️⃣ Each aligner moves the teeth incrementally and is worn for 1-2 weeks.
4️⃣ Aligners are replaced periodically until the teeth reach the desired position.
5️⃣ Once treatment is complete, retainers are used to maintain the results.

Incognito and Lingual Orthodontic Treatment

Lingual orthodontic treatment is an aesthetic method where braces are placed on the inner (tongue-side) surface of the teeth instead of the front. Incognito is a specialized lingual orthodontic system that uses custom-made gold alloy brackets for a fully personalized treatment. This technique is one of the most discreet orthodontic solutions for patients seeking invisible braces.

🦷 Advantages of Lingual (Incognito) Treatment

✅ Completely invisible – Brackets are placed behind the teeth, making them undetectable from the outside.
✅ Custom-made for each patient – Designed specifically for individual needs, ensuring a comfortable and effective treatment.
✅ Ideal for those with aesthetic concerns – Particularly popular among adults seeking a discreet orthodontic solution.
✅ Biocompatible materials for a perfect fit – Incognito brackets are made of gold alloy, ensuring an excellent fit and minimizing allergic reactions.
✅ Faster treatment with less friction – The specialized design can shorten treatment time in some cases.

Jaw Treatments (Orthognathic Surgery & Orthodontic Solutions)

Jaw irregularities can cause both aesthetic and functional issues. Jaw treatments correct misalignments of the upper and lower jaws, leading to a healthy bite, better chewing function, and an improved smile.

🦷 Jaw Disorders & Treatment Methods

1️⃣ Orthognathic Surgery (Jaw Surgery)

🔹 A surgical procedure to correct misalignment of the upper or lower jaw (protrusion or retrusion).
🔹 Combined with orthodontic treatment for optimal results.
🔹 Helps correct severe jaw deformities and breathing issues.

💡 When is orthognathic surgery needed?
✔ Prognathism (Lower jaw protrusion)
✔ Retrognathism (Lower jaw retrusion)
✔ Open bite (Teeth not making full contact)
✔ Asymmetric jaw structures

2️⃣ Orthodontic Jaw Treatments

🔹 Orthodontic appliances are used to correct narrow or wide jaw structures.
🔹 If detected at an early age, jaw growth can be guided, reducing the need for surgery.

📌 Common Orthodontic Jaw Treatment Methods:
✅ Functional Appliances – Guide jaw growth in children.
✅ Fixed or Removable Expanders – Correct narrow upper jaw problems.
✅ Splint or Clear Aligner Treatment – Suitable for mild jaw misalignments.

Impacted Tooth Eruption (Orthodontic Exposure & Traction Treatment)

Impacted teeth are those that fail to emerge into their correct position within the mouth, remaining trapped within the bone. The most commonly affected teeth are canines (cuspid teeth) and wisdom teeth (third molars). Impacted tooth eruption treatment combines surgical and orthodontic techniques to guide the affected tooth into its proper position.

🦷 How is Impacted Tooth Treatment Performed?

✅ Surgical Exposure – A minor surgical procedure is performed to uncover the impacted tooth.
✅ Orthodontic Traction (Braces or Aligners) – A bracket or attachment is bonded to the exposed tooth, and gentle forces are applied to guide it into alignment.
✅ Monitoring & Adjustments – The tooth gradually moves into its correct position over time with orthodontic adjustments.

💡 Why Treat Impacted Teeth?
✔ Prevents damage to neighboring teeth
✔ Reduces the risk of cysts or infections
✔ Improves bite alignment and aesthetics
✔ Preserves natural teeth instead of extraction

Early detection through X-rays and orthodontic consultation increases treatment success!

Cleft Lip and Palate (Orofacial Clefts)

Cleft lip and palate are congenital facial and oral malformations that occur when the upper lip and/or the roof of the mouth fail to fully fuse during fetal development. These conditions can affect a baby’s ability to eat, speak, hear, and even breathe properly.

🦷 Causes of Cleft Lip and Palate

🔹 Genetic factors – A family history of clefts increases the likelihood.
🔹 Folic acid deficiency – Poor maternal nutrition during pregnancy may contribute.
🔹 Smoking, alcohol, and drug use – Exposure to harmful substances raises the risk.
🔹 Environmental influences – Radiation or chemical exposure during pregnancy may play a role.

Orthognathic Surgery (Jaw Surgery)

Orthognathic surgery is a corrective jaw surgery performed to fix misalignment between the upper and lower jaws. It is planned in combination with orthodontic treatment to achieve both aesthetic and functional improvements in a patient’s bite and facial structure.

🦷 When Is Orthognathic Surgery Necessary?

🔹 Prognathism or Retrognathism (Lower jaw too far forward or backward)
🔹 Maxillary misalignment (Upper jaw excessively forward or backward)
🔹 Jaw asymmetry and facial imbalances
🔹 Open bite (Teeth do not fully close together)
🔹 Severe jaw narrowing or breathing problems (e.g., Sleep Apnea)
🔹 Skeletal discrepancies that cannot be corrected with braces alone

📌 Orthognathic Surgery Process

1️⃣ Orthodontic Preparation

✔ Before surgery, orthodontic treatment is used to align the teeth properly.
✔ This pre-surgical phase usually lasts 6 months to 1.5 years.

2️⃣ Surgical Procedure

✔ Performed under general anesthesia, the surgeon repositions the jawbones as needed.
✔ The upper or lower jaw (or both) may be cut, adjusted, and fixed using titanium plates and screws.
✔ The procedure typically takes 2-4 hours.

3️⃣ Recovery Process

✔ Swelling and mild pain are common in the first 1-2 weeks, but gradually improve.
✔ Normal chewing function is regained within 4-6 weeks.
✔ Complete healing can take 6-12 months.
